About the job

Basic Purpose of the role:

Responsible for providing safe, effective, evidence based & comprehensive specialized nursing care as part of the multidisciplinary team within the Home and community setting.

Key Responsibilities of the role:

Leadership

  • Demonstrates a professional respect and commitment to the care of patients, families and colleagues.
  • Demonstrates critical thinking, conflict management and problem-solving abilities.
  • Promotes an interdisciplinary team approach based on mutual respect and direct communication between health care professionals to ensure appropriate care is delivered effectively.
  • Positively participates in annual performance review and contributes to objectives and goal setting with their line manager.
  • Supervises and orientate junior staff in the home health care setting to ensure the provision of safe and effective patient care.

Clinical Practice

  • Responsible to ensure that the nursing process of diagnosis, assessment, planning, implementation, and evaluation of patient care within the home environment is in collaboration with the family, other stakeholders .
  • Collaborates with the Interdisciplinary team to provide individualized care from point of first contact throughout the episode of care, incorporating the continuum of care and comprehensive discharge planning.
  • Assume role of key worker as allocated with individual patients, coordinate all aspects of their care, re-evaluating the care plan as required, liaising with the wider multidisciplinary team as required, to complete re-assessments monthly for resubmission to respective insurance providers.
  • Set SMART goals for patients and document patient outcomes appropriately.
  • Demonstrates accountability for own professional judgement, actions, outcomes of care, continued competence and works within the local health regulatory authority Scope of Practice for Registered Nurses.
  • Demonstrates ability to identify significant changes in patient’s conditions, initiates appropriate action and escalates concern’s appropriately, whilst working autonomously within the home and community care setting.
  • Demonstrates service excellence and acts as a patient advocate to support the patients’ rights and dignity in the home at all times. Takes all reasonable steps to protect children or adults who are vulnerable or at risk from harm, neglect or abuse.
  • Administers medications, understands contraindications and manages side effects management in line with the organizations policies and procedures and as ordered by the Physician.

Knowledge & Understanding

Essential

  • Excellent verbal and written communication skills
  • Proficient with information technology platforms
  • Experience with audits and service improvements
  • Ability to work within a team and independently

  • Teaching and mentoring
  • Supervision and management

Qualification, Certifications & Experience

Qualifications & Certifications Required:

  • Bachelor of Science degree in Nursing OR
  • Associate degree in Nursing from USA or Canada (minimum 2 years course duration) and successful completion of the national exam (USA: NCLEX-RN, Canada: Licensing Exam);
  • Current National license

Desired:

  • Specialist Practitioner/Community Nursing course recognized in country of origin

Experience Required:

  • Not less than 3 years post graduate experience as a Staff Nurse with relevant experience of patients in a community setting.
